Please start by completing the below form so that our team can learn more about you and your future practice.

You may start by gathering and providing these documents to us. Please email the following  items to credentialing@neighborhoodnurse.com, and include your full name and practice name in the subject line.

​Credentialing Document Checklist

  • APRN National Certification

  • APRN State License

  • RN State License

  • DEA License

  • CSR License (if applicable)

  • Driver's license 

  • Personal W-9 form (click here to print and complete with your SSN & signature)

  • Resume, with months and years of employment

  • Malpractice insurance (if you have it, if not, see the FAQs below)

  • CAQH Username and Password

  • PECOS Username and Password

  • Availity Username and Password 

Marketing Kick-off​

Once we receive your credentialing information and associated documents listed above, we will provision your practice's professional email and will kick-off your practice's marketing. During implementation, we will spend a lot of time discussing your marketing, but to lay the groundwork, we can begin to piece together your website. Click the below link to kick off your marketing by selecting a website template, creating a professional logo, and receive next steps for content for your practice's site. Banking & Accounting

Your business will need its own bank account and accountant. Click the below button to learn how to create your business bank account, and why an accountant is vital for you as a business owner.
