  • APRN National Certification

  • APRN State License

  • RN State License

  • DEA License

  • CSR License (if applicable)

  • Driver's license 

  • Personal W-9 form (click here to print and complete with your SSN & signature)

  • Resume, with months and years of employment

  • Malpractice insurance (if you have it, if not, see the FAQs below)

  • CAQH Username and Password

  • PECOS Username and Password

  • Availity Username and Password
